2011 Fall Registration

  1. If you are a new student to Coastal Dance, please contact Terri or Brittany for class assignment and monthly payments. This way everyone is registered for the correct class placement.
  2. If you are a returning student, please select your classes. If you are unsure of your class placement or monthly payments, please contact Terri or Brittany.
  3. For multiple children, please fill out this form multiple times.
  4. Once you have completed this process and have mailed your payment, your registration is complete. Please make all checks payable to Coastal Dance Academy.

  • I understand that Coastal Dance Academy may take photographs and/or videotape the students throughout the year. I authorize Coastal Dance Academy to use these photographs and/or videotapes for publicity purposes. (Photos for the website, audition tapes and newspaper ads)

  • I have read & understand the studio rules and tuition policies reguarding tuition payments, costume payments, late fees and other fees associated with enrolling my child at Coastal Dance Academy. I know and/or have been informed to check the website weekly for important Coastal Dance Academy information.
  • I understand that Coastal Dance Academy dance instructors are not liable for any incidental injuries that may occur throughout the year. (Sprains, stress fractures, broken bones, etc.)
